Disjointed care
Part 1: Think about a time when you (or a family member) received disjointed care due to poor interprofessional communication. Briefly describe this scenario.
Part 2: As a result of this disjointed care, a job posting about what the perfect “care coordinator” would look like. What is your creative solution to increasing interprofessional communication? What will this person’s education be and what type of training will they need? Will they need to be well-versed in billing codes and communicate with insurance companies? Will they need to be able to write prescriptions? Who would be their direct employer? Where would their salary derive from and how would you demonstrate that it is not only improving ‘patient-centered care’ but also is a cost-effective approach? What will be the official job title? What other job duties need to be considered? You may use humor if feel it’s appropriate.
