My capstone change proposal is related to fall prevention in the elderly in a home setting. Please help me to discuss the highlights of my strategic plan and any modifications that need to be made. based on the strategic plan. My strategic plan includes; conducting a survey to see how healthcare providers and elderly patients who have experienced falls in homes and care facilities will view the proposed nursing intervention.

The next step is to educate the elderly patients so that they will willingly participate in the planned intervention. Educating and training the nursing staff about fall prevention.
