Accounting for Disclosures – Information that describes a covered entity’s disclosures of PHI other than for treatment, payment, and health care operations; disclosures made with Authorization; and certain other limited disclosures. Facilities must keep an accounting of released patient records. There are certain requests that do not have to be included on this accounting of disclosure. Discuss three types of disclosures that are not required to be included and why. Do not use TPO (treatment, payment or operations) in your discussion.
