Discharge planning early on in treatment

The concept of discharge planning is essential to understand, as is creating a discharge/transition plan at intake. While the plan will not be complete, having a clear objective document that helps identify when a client may be ready to leave for a lower level of care is very important in treatment planning and getting buy-in from the client. How do you think you can incorporate discharge planning early on in treatment?
