How does a neighborhood assessment help with identifying health needs for a family or community?

1)  Medical Providers-Are doctors and dentists available?-What other types of medical providers are close by?

2)  Churches and Faith-based facilities-Are their churches immediately available?-What about other faith-based organizations or worship facilities

3)  Schools-What types of schools are in the area? Are there Elementary, middle school, high school, or college?-Are religious schools available? What kinds

4)  Transportation-Does the family has a car?-Can they get to where they need to go with other means of transportation-bus, train and how far away are the stops?-Are the roads in the area in good condition?

5)  Emergency providers-How far away are the police?-How far away is the fire department?-How long would it take for a response in an emergency?

6) Culture-Are there signs of culture in the neighborhood?-What cultures live in the area?13.  Describe any health or environmental needs noted in the neighborhood.
