3 – OPERATIVE REPORT (13 errors) PREOPERATIVE DIAGNOSIS: Lesion, left upper eyelid, rule out basil cell carcinoma. POSTOPERATIVE DIAGNOSIS: Lesion, left upper eyelid, rule out basal cell carcinoma. NAME OF PROCEDURE: Excisional biopsy, lesion left upper eyelid. ANESTHESIA: Focal. COMPLICATIONS: None. INDICATIONS: This 77—year—old male patient has had a recurrent lesion in this area. Consent was obtained for incisional biopsy. She was aware of and excepted the risks, including but not limited to, anesthetic, infection, bleeding, scar, recurrence, and possible need for further management. DETAILS OF PROCEDURE: The patient was taken to the operating room where he was positioned in the prone position. A drop of Xylocaine was applied to the left eye. He was prepped with povidone-iodine. A total of 1 mL of 2% lidocaine with 1,100,000 Epinephrine was used to infiltrate the left upper eyelid. Tissue forceps and Westcott scissors were used for excision. This was sent in formalin to pathology. Dovie electric cautery was used for hemostasis. Erythromycin ointment was applied, but no patch. He tolerated the procedure well and left the operating room in good condition.
