The Clinical Population Diabetic Patients

 

1. Description of the Clinical Population

  • Diabetic patients in the hospital.

 

2. Define the Health Issue (must be related to a transition in care)

  • These patients are susceptible to getting severe Hyperglycemia and/or Hypoglycemia in the hospital
  •  Consider the following:
  • 2a . When and where does care take place
  • 2b. Identify the kinds of transitions in care the population commonly experiences 

 

3. Using the levels of the Socio-Ecological Model (SEM) address the following:

  • 3a.Individual and Interpersonal levels
  • 3b. Organizational and Community/Environmental levels
  • 3c. Society and Public Policy

 

4. Identify 2 social determinants of health that impact the population health issue

 

5. Describe the significance of SEM and its application to this issue

 

5a. the use of Continuous Glucose monitoring(CGM) in the hospital , describe its application across each of the levels of the SEM

5b. Explain how applying (CGM) across all levels of the SEM will contribute to positive health outcomes (as it relates to transitions in care)

5c.Describe stakeholders and/or organization that will work with to execute your intervention

5d. Discuss funding for your intervention, evaluation of the intervention going forward and sustainability of your intervention.

6. Discuss how Implicit Bias may affect the outcomes of the (CGM) use.
