Preliminary care coordination plan

Develop a 3-4 page preliminary care coordination plan for a selected health care problem. Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.

Introduction

NOTE: You are required to complete this assessment before Assessment 4.

The first step in any effective project is planning. This assignment provides an opportunity for you to strengthen your understanding of how to plan and negotiate the coordination of care for a particular health care problem.

Include physical, psychosocial, and cultural considerations for this health care problem. Identify and list available community resources for a safe and effective continuum of care.

As you begin to prepare this assessment, you are encouraged to complete the Care Coordination Planning activity. Completion of this will provide useful practice, particularly for those of you who do not have care coordination experience in community settings. The information gained from completing this activity will help you succeed with the assessment. Completing formatives is also a way to demonstrate engagement.

Preparation

Imagine that you are a staff nurse in a community care center. Your facility has always had a dedicated case management staff that coordinated the patient plan of care, but recently, there were budget cuts and the case management staff has been relocated to the inpatient setting. Care coordination is essential to the success of effectively managing patients in the community setting, so you have been asked by your nurse manager to take on the role of care coordination. You are a bit unsure of the process, but you know you will do a good job because, as a nurse, you are familiar with difficult tasks. As you take on this expanded role, you will need to plan effectively in addressing the specific health concerns of community residents.

To prepare for this assessment, you may wish to:

· Review the assessment instructions and scoring guide to ensure that you understand the work you will be asked to complete.

· Allow plenty of time to plan your chosen health care concern.

Note: Remember that you can submit all, or a portion of, your draft plan to Smarthinking Tutoring for feedback, before you submit the final version for this assessment. If you plan on using this free service, be mindful of the turnaround time of 24-48 hours for receiving feedback.

Instructions

Note: You are required to complete this assessment before Assessment 4.

Develop the Preliminary Care Coordination Plan

Complete the following:

· Identify a health concern as the focus of your care coordination plan. In your plan, please include physical, psychosocial, and cultural needs. Possible health concerns may include, but are not limited to:

3. Stroke.

3. Heart disease (high blood pressure, stroke, or heart failure).

3. Home safety.

3. Pulmonary disease (COPD or fibrotic lung disease).

3. Orthopedic concerns (hip replacement or knee replacement).

3. Cognitive impairment (Alzheimer’s disease or dementia).

3. Pain management.

3. Mental health.

3. Trauma.

. Identify available community resources for a safe and effective continuum of care.

Document Format and Length

. Your preliminary plan should be an APA scholarly paper, 3-4 pages in length.

5. Remember to use active voice, this means being direct and writing concisely; as opposed to passive voice, which means writing with a tendency to wordiness.

. In your paper include possible community resources that can be used.

. Be sure to review the scoring guide to make sure all criteria are addressed in your paper.

7. Study the subtle differences between basic, proficient, and distinguished.

Supporting Evidence

Cite at least two credible sources from peer-reviewed journals or professional industry publications that support your preliminary plan.

Grading Requirements

The requirements, outlined below, correspond to the grading criteria in the Preliminary Care Coordination Plan Scoring Guide, so be sure to address each point. Read the performance-level descriptions for each criterion to see how your work will be assessed.

. Analyze your selected health concern and the associated best practices for health improvement.

8. Cite supporting evidence for best practices.

8. Consider underlying assumptions and points of uncertainty in your analysis.

. Describe specific goals that should be established to address the health care problem.

. Identify available community resources for a safe and effective continuum of care.

. Organize content so ideas flow logically with smooth transitions; contains few errors in grammar/punctuation, word choice, and spelling.

. Apply APA formatting to in-text citations and references, exhibiting nearly flawless adherence to APA format.

12. Write with a specific purpose with your patient in mind.

12. Adhere to scholarly and disciplinary writing standards and current APA formatting requirements.

Additional Requirements

Before submitting your assessment, proofread your preliminary care coordination plan and community resources list to minimize errors that could distract readers and make it more difficult for them to focus on the substance of your plan. Be sure to submit both documents.

Portfolio Prompt: Save your presentation to your ePortfolio.

Competencies Measured

By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

. Competency 1: Adapt care based on patient-centered and person-focused factors.

13. Analyze a health concern and the associated best practices for health improvement.

. Competency 2: Collaborate with patients and family to achieve desired outcomes.

MBA 645 Module Five Recommendation Report Guidelines and Rubric

Overview

Millennials (born between 1981 and 1996) and Generaon Z (born between 1997 and 2015) are the two target audience groups

that are important for businesses in the entertainment category.

As a regional markeng director of the theme park, you are aware that more than 70% of your target audience falls into the

Generaon Z category. Understanding their needs, mindsets, percepons, and preferred methods of communicaon is essenal for

your business and your markeng strategy.

Considering these points, you will dra a recommendaon report that will help the chief markeng officer (CMO) and the other

markeng directors understand the importance of these major stakeholders in the markeng strategy for park reopening. Your

recommendaon report should also include the correct markeng strategies to engage Generaon Z.

Prompt

Dra a report for the CMO in the scenario and recommend markeng strategies to engage Generaon Z, or the “I-Generaon.”

Specifically, you must address the following criteria:

1. What is unique about Generaon Z in terms of markeng?

Idenfy two digital markeng approaches that work best for this generaon.

2. How do you best reach this generaon with your message focusing on brand image ?

Idenfy two social media plaorms preferred by Generaon Z. Give examples and reasons.

What is the role of bloggers or other influencers in connecng with Generaon Z? Describe and provide examples.

Do online adversements work with this generaon? Explain.

3. Describe the role of tradional methods of markeng with Generaon Z.

Which forms of tradional media can work best in influencing Generaon Z?

How will the park use these forms of tradional media in its plan?

Consider outdoor, promoonal, and guerilla markeng.

Describe the pros and cons of using TV and radio.

Guidelines for Submission

In a Word document, use double spacing, 12-point Times New Roman font, and one-inch margins. This recommendaon report

should be 1 to 2 pages in length. Consult the Shapiro Library APA Style Guide for more informaon.

An experimental Research study

Prior to beginning work on this assignment, be sure to have read all the required resources for the week. Find an experimental research study on the topic chosen in Week One for your Final Research Proposal.

You may choose to include an experimental study which was included in the literature review you used in the Week One assignment by searching the reference list for experimental research studies on the topic.

However, it is also acceptable to find and include an experimental research study on the topic that is not included in that literature review.

Identify the specific experimental research design used in the study. Summarize the main points of the experimental research study including information on the hypothesis, sampling strategy, research design, statistical analysis, results, and conclusion(s). Evaluate the published experimental research study focusing on and identifying the specific threats to validity that apply to the chosen study.

Explain whether or not these threats were adequately addressed by the researchers. Describe how the researchers applied ethical principles in the research study.

The Forces driving the Change in an organization

Prepare a 3-4 page proposal for a new software system that explains the forces driving the change in an organization , the ethical considerations behind the proposed change, the risks to the organization of not accepting the proposed change, and the benefits of accepting the proposed change.

Introduction

Organizations, such as the Joint Commission and the National Committee for Quality Assurance, collect and report extensive data on quality indicators used by the industry, government, and consumers.

The Health Insurance Portability and Accountability Act (HIPAA), passed by Congress in 1996, protects the privacy and confidentiality of patient health records.

The Office for Civil Rights (OCR) of the U.S. Department of Health & Human Services oversees this compliance and offers comprehensive support and guidance for health care leaders and organizations.

The use of health information technology and electronic health records continues to improve quality of care indicators and offer expanding career opportunities in the health care field.

However, keeping abreast of rapid advancements in technology (for both management and clinical applications), along with their ethical implications, demands increased time and attention from health care operations managers. The consequences of data security breaches can be detrimental in legal costs and loss of reputation and trust within the industry and community.

Preparation

Imagine that you work in a mid-sized clinic that is well established, having been in operation for over 25 years.

The physicians who practice in the clinic have been doing so for the same length of time, although several younger physicians have recently joined. You have been trying to get the physicians to upgrade the outdated health information technology (HIT) system for the practice.

There have been reports of compromised patient information from other organizations using the same software.

Complaints have been increasing from other health facilities and insurance companies that the software is not compatible with their electronic health records (EHR) systems. This compromises efficient and confidential sharing of patient information.

Patients have expressed dissatisfaction that the practice does not offer a patient portal to allow them direct access to their records and information and to self-scheduling features.

You feel that a personal health record (PHR) would meet the patients’ need as well as alleviate a lot of extra work for the staff.

The office and administrative staff are increasingly frustrated, as they understand how much of their daily workload duties and flow could be improved with an entirely new system.

The physicians have been resistant to change the system because it will require extensive time and money to integrate. Although it is obvious that there are multiple ethical concerns present, they are uncomfortable with managing such a large change in the practice, especially in training themselves and the staff on how to use it effectively and efficiently.

You just received a notice from the software company informing you that by the end of the year, they will no longer support that software. The company is also offering a significant discount to upgrade to their latest software system.

You must convince the group of physicians that the time to upgrade is now, but you need to position a proposal in a way that will show them the risks of not taking action.

For this assessment, you do not need to research or recommend any type of software. Rather, your task is to explain why the new software is needed in terms of the driving forces, the ethical principles involved, the risks associated with not accepting your proposal, and how the upgrade will benefit the entire organization.

Research federal regulations around the use of technology and data, HIPAA, and probable consequences should patient information be hacked. (The U.S. Department of Health and Human Services Office for Civil Rights identifies real cases of HIPAA violations on its Web site.)

Find at least three authoritative resources to use as support for your proposal.

Structure this assessment in a clear, logical order, using headings and subheadings as appropriate. Follow APA guidelines for citing and referencing your resources. Include a title page and references page.

Instructions

Prepare a proposal for the implementation of a new electronic patient health record system. In your proposal, complete the following:

· Explain the external forces that are driving the proposal. (Hint: federal regulations are not the only forces you should consider.)

· Explain the ethical considerations that are driving the proposal.

· Describe the risks and their associated ethical challenges related to not accepting the proposal. (Hint: there are many risks here. Be sure you consider the risks from multiple perspectives.)

· Explain how the proposed change will benefit and ensure ethical integrity for the organization, employees, and patients.

Additional Requirements

· Include a title page and references page.

· Number of pages: 3–4, double-spaced.

· At least three current scholarly or professional resources.

· APA format for citations and references.

· Times New Roman font, 12-point.

Note: The assessments in this course build upon each other, so you are strongly encouraged to complete them in sequence.

Competencies Measured

By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

· Competency 1: Explain how ethical theories and principles influence health care organizational management practice.

. Describe the risks and the associated ethical challenges for a health care organization related to not making changes.

· Competency 2: Describe how external forces and internal organizational factors influence ethical health care management practices.

. Explain the external forces that influence change in a health care organization.

. Explain the ethical considerations that influence change in a health care organization.

. Explain how change benefits and ensures ethical integrity for a health care organization, its employees, and its patients.

· Competency 4: Communicate ideas effectively.

. Correctly format citations and references using current APA style.

. Write content clearly and logically, with correct use of grammar, punctuation, and mechanics.

Overview

Markeng in the digital age is very dynamic; it is necessary to monitor mulple digital channels and plaorms to engage with your

target audience without missing any opportunity. Digital channels and plaorms are measurable and allow for monitoring

performance of your markeng efforts in real me.

As a regional markeng director in the course scenario, you are tasked with designing a phased markeng strategy for reopening the

park. You have already done an analysis of brand implicaons and have idenfied suitable tradional markeng methods to address

the markeng needs for park reopening. As the organizaon has chosen a blended approach (tradional plus digital) for markeng,

your CMO wants you to create a digital markeng strategy to go with the tradional markeng strategy you already determined.

Prompt

Create a narrated PowerPoint presentaon to help other directors in the scenario understand the importance of digital markeng

for brand awareness. Create speaker notes, if necessary, for each slide. Specifically, you need to address the following criteria:

1. How will you use digital markeng to enhance brand awareness ? (slide 1)

2. Idenfy two digital markeng methods that can be used for brand awareness and support with reasons. (slides 2–3)

Define short-term and long-term goals for brand awareness in the digital markeng strategy.

How will you determine the balance between the digital and tradional markeng?

How will the idenfied methods help you reach the target audience?

How will you measure the performance of your markeng efforts?

3. Use search engine opmizaon (SEO) to improve search performance. (slides 4–5)

Idenfy five keywords you will use to improve web search performance, and jusfy your strategy for keyword

selecon.

How will you redesign or rewrite the content of your website to support SEO strategy?

4. Describe the inbound markeng efforts. (slide 6)

How can they help in brand awareness?

Define the required content strategy for inbound markeng.

Guidelines for Submission

Submit a PowerPoint presentaon with 5–7 slides. Consult the Shapiro Library APA Style Guide for more informaon.
