Confidentiality is an important aspect of counseling. This means that under normal circumstances no one outside the Counseling Center is given any information — even the fact that you have been here — without your expressed written consent.

How do you maintain confidentiality in Counselling?
To maintain confidentiality, it is the counselor’s responsibility to keep the client’s records safe and appropriately secured. Records should be locked away where only the counselor can reach them. Protect records at home. It is important that you lock away documents at home as well as at the office.

Study of risk factors for lung cancer

Cognitively impaired elder assessment

Cognitively impaired elder assessment. The registered nurse reports that when the nurse aids attempts to get him out of bed, he actively resists and strikes out. He also screams when being showered, especially when his lower extremities are washed, and when being dressed. He has been moved to a private room in the facility due to his behavioral changes disturbing his roommate. The registered nurse reports that he has a large ulcer on the medial aspect of his left foot as well as a small ulcer on the malleolus. His right heel is reddened and soft to the touch. The patient’s daughter is present at the meeting and asks the registered nurse whether she thinks her father is in pain. The registered nurse responds that she attempted to administer the pain scale but that he was not able to respond.

Pediatric primary care

Aiden is a 13-year-old white male seen in pediatric primary care for concerns regarding behavior. Mom states she “doesn’t know what to do anymore”. Aiden is failing all of his classes with the exception of gym and music. Aiden does well on test scores but does not turn in homework, even after he has completed it. Mom created a binder for him to organize all of his work in, and he has an assignment notebook where he is supposed to write down his assignments. He doesn’t often use the notebook and uses the binder properly about 50% of the time. Aiden is in 7th grade and at risk of not being able to stay in his school next year for 8th grade. Aiden is a very 13-year-old, and her mom says his teachers feel that way as well. Aiden has stayed on the same low dose of Focalin XR 10mg for 4 years, and one low dose, short-acting Adderall 5mg for homework in the evenings. Aiden expresses an increase in anxiety. Mom states that his parents separated 2 years ago, but are now in the process of selling the house Aiden has lived in for most of his life and she is concerned that this is contributing to his anxiety/decrease in school performance.
