Nursing diagnosis

Hi,

I am after assistance with a ‘nursing diagnosis‘ with ‘related to‘ and ‘evidenced by‘ statements please. The problem is regarding an older person (78yrs) and his sleep hygiene – he spends 8-9 hours in bed (usually 1 hour pre sleep time reading, listening to classical music or watching sport on TV/iPad). They wake 3-4 times a night, visit the bathroom 1-2 times, but usually have trouble getting back to sleep. He estimated a total of 5-6hrs actual sleep time. He has prescribed sleep medication (low dose) that he usually takes approx. 2 x per week.

 

He doesn’t have any associated medical conditions that could be effecting this. He has reported well managed stage 3 kidney disease and well managed elevated cholesterol, plus osteoporosis in R. knee (some mild pain occasionally), but those issues don’t seem to relate to the sleep disturbance, so I am struggling to come up with the ‘related to‘ statement of the nursing diagnosis.

 

If you could provide a nursing diagnosis and any suggestions that would be very helpful.
