Decrease in the frequency

Ms. Hughes is at her wits’ end with Oran and his clowning around. She has tried scolding them, moving his seat, subtracting points, and giving him extra assignments. There has been no decrease in the frequency of his behavior. She decides to ignore the behavior and put it on extinction, understanding and expecting that the behavior will increase before it decreases. Oran continues the behavior, it continues to be disruptive and makes the other students laugh and talk back to him. She studiously ignores each occurrence and continues lessons and group work through the behavior. But after 2 weeks without any decrease in the behavior, she is giving up.

Memorial Hospital

The pericardial cavity

GROSS AND MICROSCOPIC DESCRIPTION

GROSS EXAMINATION   The body is that of a 64-year-old male. The body was opened by means of a midline incision showing the pleural and peritoneal cavities.  The organs were removed en toto by the Rokitansky method.

CARDIOVASCULAR SYSTEM:   The pericardial cavity contains a minimal amount of clear fluid. The heart appears enlarged and measures 13.0 x 10.0 x 8.0 cm and weighs 480.0 grams. Cut sections show a thickened left ventricular wall measuring 2.5cm, with a focus on a reddish area in the anterior ventricular wall, surrounded by white, firm tissues and cardiac muscles. Coronary arteries show evidence of calcification. Microsections show edema, hemorrhage, infiltrates of neutrophils, and mottling of the cardiac myocytes.  Other areas of show bands of fibrotic tissues traversing the cardiac myocytes. Coronary arteries are atherosclerotic.

RESPIRATORY SYSTEM:   The lungs are reddish black and spongy.   The right lung measures 22.0 x 11.0 x 8.5 and weighs 300.0 grams while the left lung measures 23.0 x 12.5 x 7.5 and weighs 275 grams. Cut sections show a moderate amount of mucoid material within the airway lumen. There are no gross masses noted. Microscopic sections of both lungs show carbon dust and hemosiderin-laden macrophages noted amidst pools of edema fluid, blood, and congested vascular channels. Dilated alveoli with damaged alveolar walls are also seen. No evidence of malignancy was noted.

LIVER:  The liver is enlarged, yellowish-brown, and smooth, measuring 27.0 cm x 24.5 cm x 14.0 cm and weighing 2100.0 grams.  Cut sections show a yellowish-brown, greasy surface. Microsections of the liver show sheets of hepatocytes arranged in single-cell thick plates separated by vascular sinusoids filled with red blood cells. These hepatocytes exhibit lipid vacuoles within the cytoplasm. There are patches of lymphocytic infiltrate and foci of hepatocellular necrosis around the central veins.

SPLEEN:   The spleen is fairly ovoid, bluish, doughy tissue measuring 11.0 cm x 10.0 cm x 4.0 cm and weighs 180 grams. Cut sections reveal a fairly solid reddish-brown cut surface. Microsections show the spleen shows well-delineated red pulp and white pulp with congested splenic cords filled with red cells.

PANCREASThe pancreas presents as an elongated to ovoid, broad, tan to yellow tissue with a fairly smooth surface measuring 21.0 x 5.0 x 3.5 cms and weighing 115. grams. Cut sections show a yellowish-tan, lobulated surface. Microsection shows acinar glands in lobular architecture and is interrupted by islets of Langerhans. Vascular congestion is also seen.

KIDNEYS  The right and left kidneys are covered by abundant perinephric fat and measure 10.7 x 5.0 x 3.5 cm, and 11.0 x 6.5 x 5.5 cm, respectively. Cut sections reveal reddish-brown solid parenchyma with a prominent cortico-medullary junction. The pelvicalyceal system is intact. Microsections of both kidneys and the pelvicalyceal system show essentially normal findings. Vascular congestion is also seen.

 

GASTROINTESTINAL SYSTEM:   The stomach measures 22.0 x 14.0 x 6.5 cm. The mucosa appears light tan with prominent rugal folds and contains brown fluid amounting to 150.0 ml. There is no ulceration or mass noted. The small intestine consists of a pinkish-tan light-tan tissue with smooth serosa. Likewise, the colon measures 220 x 8.0cm and shows unremarkable gross findings. The appendix is absent. Microsections of the gastrointestinal tract show intact mucosa with focal patches of lymphocytic infiltrates.

chronic cough

A 50-year-old woman has been experiencing a chronic cough for the past two months. She is a two-pack-a-day smoker and has been for the past 30 years. She sees her PCP for the cough and is given an order for a chest x-ray at the local hospital. The x-ray report states “nodule in the lower lobe of the right lung, worrisome for malignancy.” The PCP refers her for a biopsy of the nodule at the ambulatory surgery clinic, which substantiates the diagnosis of cancer in the pathology report. Two weeks later, the same physician who did the biopsy performed a right lower lobectomy. Prior to discharge after surgery, the patient develops an infection, and the infectious disease specialist is asked to evaluate her and recommend the appropriate treatment course. Upon discharge, she receives care in her home for the next two weeks. Four weeks after surgery, radiation therapy is initiated, but her condition continues to deteriorate over the next few months with metastasis to the brain noted, at which point she decides to receive only palliative care.

E.coli outbreak

The outbreak that I want to investigate is the E.coli outbreak. The purpose of these questions is to verify the diagnosis

and defining & identifying cases of the outbreak. I would like some references for each question as I will be able to read the sources.

  1. What steps will you need to take to be able to speak with patients?
  2. What barriers might you run into in accessing patients? (Examples: Did they die? Do you speak the same language? Are they willing to speak to you? Are there legal issues you need to account for? This is not an all-inclusive list of possibilities.)
  3. What laboratory tests are needed? For example, would PCR testing be appropriate in this situation? (Don’t forget to think about tests appropriate for each part of the epidemiologic triad.)

Scientific communication

Identify the principles of scientific communication. Explain the ethical implications in human research as it applies to the allied health care professional. Analyze the ethical issues around communication when conducting research in the allied healthcare field. Describe how you would communicate effectively and appropriately with patients, families, and health care providers within the scope of practice. Definition of resilience

The definition of resilience as said by (Kester, 2018) is the ability to face an adverse situation, remain focused, and maintain optimism for the future. This is a perfect statement to summarise what as nurses we all must achieve no matter how dire the situation is. Steps that could be taken in my practice setting to achieve more resilience would be to make stronger bonds among peers, leading to increased communication and strength as a group which leads to the ability to maintain optimism for the future. The (ANA, 2015) states that a healthy work environment is needed to support overall job satisfaction and personal well-being.
