• Describe the termination process and plan you have developed with your client, including relevant ethical standards that informed the process, and indicate whether the client will end the services you provided with the agency or whether services will be transferred to another professional as part of the transition process.
• Describe your client’s progress with respect to the intervention goal you established and how you measured this progress.
• Describe your follow-up plan with the client, including the support services that are needed to ensure success of your intervention goals and evaluation measures.
• Describe the inter-professional collaboration efforts you included as part of work with your client, including advocacy efforts.
