Background: Addiction can damage the brain in different ways, including neurotoxicity, traumatic brain injury, and stroke. One part of the brain particularly vulnerable to damage is the frontal lobe, which is involved in executive functions (EFs). There are three ‘core’ executive functions; inhibition, working memory, and cognitive flexibility (also referred to as set shifting and/or switching) and these are vital for everyday behaviours such as reasoning, problem solving, and planning. EFs impact upon all aspects of human life, including communication. Executive functions that support social communication are often impaired in individuals with damage to the frontal lobes. Much of our knowledge about the role of the frontal lobe is based on research in those with frontal lobe damage, such as those with aphasia related to a stroke.

The patient was against surgical intervention and opted to try manual distortion

The patient is a 22-year-male with left testicular pain and scrotal swelling over the past three hours. No dysuria, no penile discharge, no fever and is able to void well. No trauma to the groin. Pain 5/10. The physician orders an ultrasound. It demonstrates a testicle that is free-floating in the scrotum with the spermatic cord looped once around the middle of the testicle. The physician diagnosed testicular torsion. The patient was against surgical intervention and opted to try manual distortion. The patient was warmed with hot packs to loosen the scrotal skin, and the physician manipulated the testicle until the spermatic cord felt loosened. Re-ultrasound confirmed the proper position, and the patient’s pain was substantially relieved. The patient was sent home with a list of precautions and instructions to return with any increase in pain.
